RadNet is honored to expand their relationship with Local 810, United Wire Metal and Machine. All their providers participate with Magnacare.  As a courtesy to our members we will waive the co-payment if you use their facilities.

WHERE CAN I GET MY HEALTH TESTS DONE?

Your doctor may prescribe certain tests to be done that need prior approval from your Fund. Always refer to your Summary Plan Description (SPD) for detailed information about your plan.

Diagnostic services are a broad range of tests that are essential to the basic management of patient care, allowing physicians to detect disease earlier, make diagnoses, prescribe therapies, and monitor patient results. If your doctor prescribes you a diagnostic test that requires radiology or blood work you must use an in-network free-standing facility.

Where do I find an ‘in-network, private free-standing facility’?

Go to Magnacare.com and read below, or Call (866)-281-0364 and follow their instructions.

  1. Click “Find a Health care provider”
  2. Click “Local 810 PPO”
  3. For X-Ray/Ultrasound testing – Click “Radiology Centers
  4. For Blood/Lab tests – Click “Laboratories
  5. Contact the provider closest to your home/work for an appointment.
  6. Bring prescription from your doctor and just pay co-pay.

  • All diagnostic tests such as x-rays, bone density, echocardiogram, mammography, echocardiogram, ultrasound, and sonograms must be done at participating labs, not at a hospital or a doctor’s office.
  •  Major participating labs are Quest, Lab Corp of America, Bio-Reference Lab, Sunrise Lab, etc.
  •  Make sure you advise your doctor to send your specimen to a participating lab.
  •  Any MRI, Endoscopy, Colonoscopy, PET or CAT Scans must be pre-approved.
  •  Blood tests can only be performed in a doctor’s office during a medical emergency.

These tests will be totally and completely paid for by your Plan if you utilize a in-network, free-standing diagnostic center. Tests done in a hospital setting will not be covered.

¿DÓNDE SE PUEDE IR PARA HACERME  MIS PRUEBAS DE  DIAGNOSTICO?

El médico puede prescribir ciertas pruebas a realizar que necesitan aprobación previa de su fondo.  Consulte siempre a su libro de descripción Resumen del Plan (SPD) para obtener información detallada acerca de su plan.

¿Qué son los servicios de diagnóstico y cómo puedo encontrar proveedores que son participantes?

Servicios diagnostico son una amplia gama de pruebas que son esenciales para la gestión básica del cuidado del paciente, permitiendo a los médicos a detectar la enfermedad antes, hacer diagnósticos, prescribir tratamientos y seguimiento de resultados de los pacientes. Si su médico le prescribe una prueba de diagnóstico que requiere trabajo de radiología o sangre se debe utilizar consultorios  diagnosticos que son participantes con su Plan y NO ESTEN EN EL HOSPITAL.

  1. Click en ‘Encontrar un medico’
  2. Click “Local 810 PPO”
  3. Para las pruebas de rayos X/ecografía – click “Radiology Centers”
  4. Por sangre y/o exámenes de laboratorio – click “Laboratories”

  • Todas las pruebas diagnósticas como rayos x, hueso densidad, ecocardiograma, mamografía, ecocardiograma, ultrasonido, y ecografías deben realizarse en los laboratorios participantes, NO EN  UN HOSPITAL O UN CONSULTORIO MEDICO.
  • Principales laboratorios participantes son Quest, Lab Corp de América, laboratorio de Bio-Reference, Sunrise Lab, etc.
  • Asegúrese de que usted aconseja su medico que le  envie su muestra a un laboratorio participante.
  • Cualquier resonancia magnética, endoscopia, colonoscopia, PET o computarizadas deben ser previamente aprobados.
  • Exámenes de sangre sólo puede realizarse en el consultorio de un médico durante una emergencia médica.

Estas pruebas se pagaran totalmente y completamente por su Plan. Si usted utilizan un centro de diagnóstico que son participantes. Las pruebas hecha en un hospital no se cubrirán.
