Please be advised that as per the IRS Notice 2020-76, Form 1095-B will not
be mailed to you in 2024.
Any participant of the United Wire, Metal & Machine, Health & Welfare Fund may receive a copy of their 2024 Form 1095-B upon request only.
You can request a copy by any of the following methods:

Attention New York Essential
New York is now offering COVID-19 testing to certain essential workers, including workers at healthcare facilities and essential workers who have contact with the public.

Important Information Regarding Your Welfare Fund Benefits

To All Active Participants and COBRA Beneficiaries:
This notice contains important information concerning benefits provided by the Welfare Fund. Please attach this letter to your Summary Plan Description (SPD). It should be read and retained with your SPD for future reference.
ADDITIONAL BENEFITS TO THE WELFARE FUND
Effective for services received on or after April 1, 2020, the Board of Trustees elected to improve benefits to the Plan with the addition of nutritional counseling. The Plan will now cover up to two (2) nutritional counseling visits per participant per year, up to $150 per visit In-Network.
As such, the following paragraph is added to the SPD under the section titled “Medical Benefits”:
“Services of an In-Network Registered Dietician, Licensed or Certified Nutritionist or Certified Nutrition Specialist limited to 2 visits per participant per Plan year, up to $150 per visit.”
Previously under the SPD, there was no coverage for nutritional counseling.
